The callback form asks for a name and a telephone number. Nothing further is requested, and no clinical or medical information is collected through this website.

How it is used

Your details are passed to my practice so that my secretary can telephone you. They are not used for any other purpose, not added to a mailing list, and not shared with any third party.

How long it is kept

Only for as long as is needed to contact you. Nothing is retained on this website.

If you go on to become a patient, your medical records are held separately and retained in line with the requirements that apply to private medical records.

Your rights

You may ask what information I hold about you, ask for it to be corrected, or ask for it to be erased. Requests can be made using the contact details on the home page.

Complaints

If you are not satisfied with the way your information has been handled, you may complain to the Information Commissioner's Office, the United Kingdom supervisory authority, at ico.org.uk.
